Job Description

The Family Medicine / General Internal Medicine Physician is responsible for providing medical examinations, diagnosis, and treatment in patients' homes. The role focuses on ensuring continuity of care and delivering compassionate, patient-centered healthcare experiences for patients and their families.

Key Responsibilities

• Conduct medical examinations, diagnose conditions, prescribe medications, and provide appropriate medical interventions at patients' homes.

• Manage chronic conditions such as hypertension, diabetes, COPD, heart failure, and other common chronic diseases.

• Conduct regular health screenings and identify potential health risks at an early stage.

• Provide preventive care, including vaccination, health screening, and lifestyle counseling.

• Educate patients and their families on disease management, medication use, and home care.

• Collaborate with nurses, technicians, and the care team to develop and implement individualized treatment plans.

• Maintain complete and accurate electronic medical records.

• Provide telemedicine consultations when necessary.

• Participate in developing clinical procedures and contributing to service quality improvement initiatives.

Requirements

• Medical degree in Family Medicine or General Internal Medicine, with a valid Family Medicine Medical Practice License.

• Minimum of 54 months of clinical medical practice experience.

• Minimum of 3 months of experience working as a Family Physician/ Family Medicine Doctor. 

• Preferred: Prior experience working in hospitals or clinics. 

• Excellent communication and active listening skills, with a compassionate, attentive, and respectful attitude toward patients.

• Strong ability to work independently, proactively, and handle clinical situations effectively in a home healthcare environment.

Job Description

Deliver a wide range of nursing services at patients' homes, working independently and proactively handling emergencies in line with established protocols, while developing and standardizing nursing procedures and training the team. Work closely with physicians to ensure quality, safety, and a positive patient experience.

Key Responsibilities

1. Home-Based Nursing Care

• Collect initial patient information, assess patient condition, measure vital signs (blood pressure, temperature, pulse, SpO2, etc.), and recognize early warning signs.

• Perform basic and advanced nursing procedures as prescribed by physicians, including injections, IV infusions, wound care and dressing, specimen collection, and other home nursing procedures.

• Guide patients and caregivers on medication use, care, and health monitoring according to medical instructions.

• Work independently at patients' homes; proactively manage emergencies according to protocol, escalate promptly to physicians, and arrange referral when needed.

2. Clinical Procedures & Quality

• Develop, review, and update home nursing procedures, emergency response protocols, infection control, and sterilization practices.

• Comply with procedures and ensure medical records and monitoring documentation are complete and accurate.

• Handle complex cases and patient feedback or complaints related to nursing services.

3. Training & Development

• Develop training programs and materials; train, guide, and assess the clinical competency of nurses, new staff, and part-time collaborators.

• Stay updated on medical knowledge and new clinical procedures and ensure consistent implementation across the team.

4. Supplies & Equipment

• Prepare, clean, and sterilize equipment before and after each visit; monitor quantity and expiry dates of medications and supplies, and propose procurement when needed.

5. Coordination 

• Work closely with physicians and the care team to ensure continuous and safe delivery of treatment and care plans.

Requirements

• Bachelor's degree in Nursing.

• Valid Nursing Practice License.

• Minimum 5 years of clinical experience, preferably in hospitals, clinics, or home care.

• Proficient in a broad range of nursing procedures and able to deliver diverse home care services.

• Able to work independently and handle emergencies proactively while strictly following protocols and clinical guidelines.

• Experience or capability in developing procedures and training colleagues.

• Highly responsible, careful, detail-oriented, honest, with strong communication skills and a friendly, professional attitude toward patients and families.
